Referrals Checklist

What constitutes a valid referral under Better Access?

In addition to a MHTP, we must receive the following information either on the MHTP or in a separate referral letter.

• Referral Date
• Patient's Name
• Patient's Date of Birth
• Patient's Address
• Diagnosis
• Request for Services under Better Access
• Number of Sessions Requested
• Referring Practitioner's Medicare Provider Number
• Practitioner's Signature
